What's the best thing to do with old SIM's that no longer work for whatever
reason? Just throw them out? Do the carriers expect them returned?

I am pretty sure they are OTP.

Yep, but destroy it first in case they do work sometimes.

According to the SFOA, you never own the SIM. But they dont want them back.
Keep them for nostalgia ;-)
