Two or three channels seem to be letterboxed AND pillarboxed at the same
time, leaving a square in the middle of the TV with the content. I have
set the box for 4:3 TV and Widescreen output - most channels display in
4:3 or in widescreen as appropriate, but others do not.

You do not say whether your TV is wide screen or 4:3.
On the widescreen channels, if your STB is set to 16:9 letterboxing, any 4:3
programmes will be pillarboxed within the letterboxed 16:9 frame. The
pillarboxing is in the broadcast transmission. In the transition to
widescreen, some widescreen programmes are being broadcast in 4:3 with 14:9
or 16:9 letterboxing, such as Showtime.

That does explain it, so Fox8 is shown widescreen - but a 4:3 picture is
being broadcast - giving me a pillerboxed imaged widescreened for
display on my TV.
